Band 6 Mental Health Practitioner – Make a Difference in Community Mental Health Care

A Day in the Life in Community Mental Health:

As a Band 6 Mental Health Practitioner working in the community, you’ll manage a caseload of service users and provide direct care through mental health assessments, care planning, and ongoing intervention. Your mornings may begin with team handovers and urgent case discussions. From there, you’ll conduct home visits or clinic appointments, liaise with families and carers, and provide evidence-based support to those experiencing mental health challenges.

You’ll coordinate multidisciplinary input, working closely with psychologists, social workers, and GPs to ensure cohesive and comprehensive care. The role involves proactive risk assessment and crisis management, as well as supporting recovery-focused pathways. By afternoon, you’ll be reviewing care plans, documenting progress, and planning follow-up actions to ensure continuity of care. Your contribution will be pivotal in enabling individuals to maintain independence and manage their mental health in the community.


Requirements and Qualifications:

  • Registration: Current and valid registration with the NMC

  • Experience: Proven experience in community mental health settings

  • Skills: Excellent communication, risk management, and clinical assessment abilities

  • Teamwork: Strong collaboration skills within multidisciplinary teams

  • Commitment: A passion for supporting individuals with complex mental health needs
