We have a fantastic opportunity for a Band 6 CPN to join a supportive Adult Community Mental Health Team in Hereford.

• Role: Band 6 CPN – Adult CMHT

• Location: Hereford

• Duration: 3-month initial contract (likely extension)

• Start date: ASAP

• Schedule: Monday to Friday, 9am – 5pm

• Hourly rate: £25 -£26 p/h

What You’ll Be Doing:

You’ll be working within an Adult CMHT, managing a caseload of service users with complex mental health needs. Duties include:

• Comprehensive mental health assessments and care planning

• Risk management and safeguarding

• Liaising with internal MDTs and external agencies

• Supporting clients with recovery-focused interventions

• Contributing to CPA reviews and clinical documentation

Requirements:

• Registered Mental Health Nurse (NMC)

• Minimum 1 year of CPN or CMHT experience

• Enhanced DBS

• Car driver preferred due to community visits

• Ability to work both autonomously and within a multidisciplinary team

Working Hours & Contract:

• Full-time hours, Monday to Friday

• 3-month rolling contract, with strong likelihood of extension
