Join our team as a Forensic Social Worker and play a key role in improving the lives of patients preparing for discharge from secure inpatient settings. In this role, you will:
perform comprehensive risk assessments, including risk of reoffending
create and implement risk management and discharge plans
deliver evidence-based care coordination to support recovery
collaborate with multidisciplinary teams, local authorities, and safeguarding boards
maintain accurate patient records and produce formal reports for panels and tribunals
To be successful in this role, you will need:
a current and valid SWE registration
experience working with adults in secure or forensic inpatient settings
strong communication, safeguarding, and care planning skills
the ability to work independently and within a multidisciplinary team
a commitment to delivering high-quality, person-centred care
This is a full-time, Monday to Friday role from 9am to 5pm. The contract is for 3 months, with a strong possibility of extension based on performance and service needs.
