Role Overview

The successful clinician will hold a defined caseload, carrying out comprehensive assessments, risk reviews, formulation, and ongoing care planning. You will work closely with a multidisciplinary team, contributing to safe and effective delivery of community mental health services.

Key Responsibilities

• Conduct holistic assessments of older adults presenting with mental health needs

• Deliver person-centred interventions and structured care plans

• Manage and prioritise a community caseload

• Complete risk assessments and implement appropriate risk management strategies

• Work collaboratively with the MDT, carers, and external agencies

• Maintain accurate, timely clinical documentation

Requirements

• Registered Mental Health Nurse (NMC)

• Previous CMHT / community experience (older adults preferred)

• Strong risk assessment and caseload management skills

• Ability to work autonomously and collaboratively within an MDT

• Enhanced DBS and compliance documents (we can support with this)
