Hunter Mental Health is supporting a well-established NHS team within Kent and Medway NHS and Social Care Partnership Trust (KMPT) with a fantastic opportunity for a Band 6 Mental Health Practitioner to join their community mental health team.

Role: Band 6 Mental Health Practitioner (RMN, OT, or Social Worker)
Location: KMPT – Medway & Swale Community Mental Health Team
Duration: Ongoing contract
Schedule: Monday to Friday, 9am–5pm (37.5 hours/week)
Start Date: ASAP
Rate: £27 per hour

What You’ll Be Doing:

  • Undertaking initial meetings, including use of the DIALOG+ assessment tool

  • Carrying out core assessments and trusted assessments for service users with complex needs

  • Coordinating comprehensive mental, physical, and social assessments

  • Developing holistic, person-centred care plans with the service user and their carers

  • Delivering, coordinating, and evaluating safe, recovery-focused interventions

  • Working collaboratively within a multidisciplinary team to support recovery pathways


Requirements:

  • Qualified RMN, OT, or Social Worker with valid registration (NMC/HCPC/Social Work England)

  • Minimum of 2 years post-registration experience in mental health

  • Experience working in a community mental health team

  • Skilled in clinical assessments across psychological, physical, and social domains

  • Confident using RIO and DIALOG+

  • Ability to offer carer/family support in distressing situations

  • Must be able to travel independently as part of the role


Working Hours & Contract Length:

  • Monday to Friday, 9:00am – 5:00pm (No nights, no bank holidays)

  • 37.5 hours per week

  • Ongoing contract
