CHC Nurse Assessor | Band 6
Location: West Midlands area
Rate: £300 per day
Working Pattern: Monday to Friday, 9:00am – 5:00pm
Team: All Age Continuing Healthcare (AACHC)

What You’ll Be Doing

  • Conduct thorough assessments, reviews, and monitoring of care for clients funded via NHS Continuing Healthcare and Funded Nursing Care

  • Act as Case Manager to facilitate, plan, and evaluate client care in collaboration with families and professionals

  • Maintain strong relationships with health, social care, and independent sector colleagues

  • Provide professional advice, training, and mentorship to colleagues where required

  • Participate in service development, clinical audits, and research to improve practice and patient outcomes

  • Ensure all assessments are completed in line with national frameworks, safeguarding, and statutory guidance

Requirements & Qualifications

  • Registered Nurse with current NMC registration or equivalent

  • Educated to degree level or equivalent experience in a specialist area

  • Previous experience conducting Continuing Healthcare assessments for adults with complex needs

  • Strong knowledge of multi-disciplinary team working, safeguarding, and clinical governance

  • Excellent communication, negotiation, and conflict management skills

  • Ability to manage complex caseloads and work autonomously

  • Experience in providing advice and support across health, social care, and independent sectors

Desirable

  • Master’s in healthcare or related area

  • Experience in leadership roles and cross-boundary collaboration

  • Knowledge of commissioning arrangements, DOLS, and SEND agenda

Working Hours & Contract Duration
Monday to Friday, 9:00am – 5:00pm
Contract: 3–6 months ongoing, ASAP – 4 weeks start
