• Job Title: Band 6 CPN / Social Worker
• Location: Hemel Hempstead (The Marlowes, HP1 1LD)
• Duration: 3-month contract
• Schedule: Monday to Friday, 09:00–17:00
• Start Date: ASAP
• End Date: 3 months from start
• Rate of Pay: £26 per hour

What You’ll Be Doing:

Join our team as a Community Mental Health Practitioner and play a key role in supporting adults in the community with complex mental health needs. In this role, you will:

• Perform comprehensive patient assessments
• Create and implement personalised care plans
• Deliver evidence-based treatments to optimise patient outcomes
• Collaborate with multidisciplinary teams to provide holistic care
• Maintain accurate patient records

Requirements & Qualifications:

To be successful in this role, you will need:

• A current and valid NMC or Social Work England registration
• Experience working with adults in community mental health settings
• Strong communication and interpersonal skills
• The ability to work independently and within a multidisciplinary team
• A commitment to delivering high-quality patient care

Working Hours & Contract Duration:

This full-time role runs Monday to Friday, 09:00–17:00, for an initial 3-month contract, with a strong likelihood of extension based on performance and service needs.
