Join our Community Mental Health Team (CMHT) as a Band 6 CPN, providing vital support to adults with complex mental health needs. Your key responsibilities will include:
✔ Managing a caseload of service users, providing assessments and care planning.
✔ Delivering recovery-focused interventions to support service users in the community.
✔ Conducting risk assessments and crisis management to ensure patient safety.
✔ Working collaboratively with multidisciplinary teams, families, and carers.
✔ Administering medication management and monitoring mental health stability.
✔ Supporting transitions between services and signposting to relevant resources.
To be successful in this role, you will need:
✔ Current and valid NMC registration as a Mental Health Nurse (RMN).
✔ Experience working in a Community Mental Health Team (CMHT).
✔ Strong assessment, risk management, and crisis intervention skills.
✔ Excellent communication and teamwork abilities.
