About the Role

Hunter Gatherer Mental Health is working in partnership with Swindon and Gloucestershire Mind, an organisation with a bold mission to reshape mental health and emotional wellbeing services across the community.

This role supports individuals aged 16+ with Autism and mild to moderate mental health needs, working at pre- and post-assessment stages. You’ll deliver collaborative, person-centred support through one-to-one interventions, group work, and psychoeducational sessions, empowering people to improve emotional wellbeing, independence, and community integration.

You’ll manage your own caseload, co-produce wellbeing plans, complete risk assessments, and work closely with local agencies, NHS partners, and third-sector organisations.


A Day in the Life

  • Deliver person-centred one-to-one and group wellbeing interventions

  • Receive and respond to referrals as directed by the service lead

  • Manage a caseload, including risk assessments and multi-agency liaison

  • Co-produce wellbeing and support plans aligned to individual goals

  • Use evidence-based tools such as the Outcome Star and 5 Ways to Wellbeing model

  • Deliver psychoeducational and peer-support groups as part of post-diagnostic support

  • Support individuals awaiting autism assessments in collaboration with the AWP Autism Team

  • Promote independence, community engagement, and emotional resilience

  • Maintain accurate records, monitor outcomes, and contribute to KPI targets


Key Responsibilities

Person-Centred Autism Support

  • Provide consistent, time-appropriate emotional, practical, and wellbeing support

  • Conduct safeguarding and risk assessments using the UK Mental Health Triage Scale (UK MHTS)

  • Carry out regular wellbeing reviews and outcome monitoring

Multi-Agency Collaboration

  • Build and maintain strong relationships with statutory and voluntary organisations

  • Signpost individuals to relevant internal and external support services

Service Development & Delivery

  • Deliver and promote psychoeducational groups and peer-support sessions

  • Support service development and community outreach

KPIs, Data & Reporting

  • Record outcomes accurately using the Views database (or alternative systems)

  • Meet monthly targets for wellbeing plans and reviews

  • Ensure individuals’ voices are upheld through advocacy and inclusive practice


Requirements & Qualifications

  • Experience supporting individuals with Autism and mental health needs

  • Background in health, social care, or the voluntary sector

  • Strong understanding of safeguarding, confidentiality, and recovery-focused practice

  • Ability to work independently and collaboratively within a multidisciplinary setting

  • Confident with caseload management, documentation, and outcome monitoring

  • Immediate availability preferred


Working Hours & Contract Duration

  • 22–35 hours per week

  • Flexible scheduling available

  • Locum contract with potential for hybrid working
