What is District Nursing?

District nursing is a specialist community nursing practice focused on delivering clinical care to patients in their own homes and community settings. District nurses manage complex, often high-acuity caseloads that include housebound patients, those with serious long-term conditions, patients receiving end-of-life care, and individuals requiring clinical interventions that would otherwise require a hospital visit.

District nursing is one of the most autonomous clinical roles in nursing. Practitioners work largely independently – visiting patients alone, making real-time clinical decisions without direct on-site medical supervision, and managing deteriorating patients in the home environment. This demands strong clinical judgment, experience with complex presentations, and confidence in managing risk.

There is an important distinction within the district nursing workforce. A qualified District Nurse holds the Specialist Practitioner Qualification (SPQ) in District Nursing – a post-registration degree-level qualification that qualifies them to lead a district nursing team and take the most complex caseload responsibilities. Community staff nurses working within district nursing teams are registered nurses (RGN) with community experience, but without the SPQ. Both roles are in demand and we place both.

District Nursing Clinical Responsibilities:

District nurses and community staff nurses within district nursing teams are typically responsible for:

  • Complex wound management – leg ulcers, pressure sores, post-surgical wounds, specialist dressings
  • Medication administration – subcutaneous and intravenous therapy, syringe drivers for end-of-life patients, insulin management
  • Catheter care – urinary and suprapubic catheter insertion, maintenance and troubleshooting
  • Palliative and end-of-life care – symptom management, syringe driver set-up and monitoring, Gold Standards Framework, family support
  • Long-term condition management – diabetes, COPD, heart failure, MS monitoring in the home
  • Post-operative and post-discharge care – monitoring recovery, managing surgical wounds, rehabilitation support
  • Continence assessment and management
  • Assessment, risk stratification and escalation of deteriorating patients
  • Coordination with GPs, social care, hospice, specialist nurses and other community services

Qualifications We Look For:

Every district nurse or community staff nurse we place holds:

  • Active NMC registration (RGN – Adult Nurse) with no conditions or restrictions
  • Relevant post-qualification community nursing experience – most district nursing locum roles require at least 12 months’ recent community caseload experience
  • Enhanced DBS and current mandatory training including safeguarding, infection control and manual handling
  • A full UK driving licence and access to a vehicle – essential for all district nursing roles
  • For qualified District Nurse (team leader/caseload holder) roles: the Specialist Practitioner Qualification (SPQ) in District Nursing

District Nurse Pay Rates (2025/26):

Community staff nurses working within district nursing teams typically earn £24–£30 per hour as a locum, reflecting the autonomous and complex nature of home-based caseload work. Qualified District Nurses with the SPQ in leadership or caseload holder roles typically earn £30–£38 per hour at Band 6, and Band 7 District Nurse team leaders or clinical leads earn £35–£45 per hour.

Palliative care-heavy caseloads, out-of-hours district nursing, and London or high-cost area placements typically attract the higher end of these ranges.

For permanent salaried roles, Band 5 runs from £31,049 to £37,796 (2025/26); Band 6 from £38,682 to £46,580; Band 7 from £47,810 to £54,710.

Frequently Asked Questions:

  • Do district nursing locum roles require the SPQ in District Nursing? It depends on the role. Team leader and caseload holder roles typically require the SPQ. Many community staff nurse positions within district nursing teams do not – they require experienced community nursing practice rather than the specialist qualification. We’ll always be specific about what each role requires.
  • Is a driving licence essential? Yes, without exception. All district nursing roles involve home visiting across a geographical patch. A full UK driving licence and access to a vehicle are a non-negotiable requirement for every district nursing placement we make.
  • How is district nursing different from community nursing? District nursing is a specific specialism within community nursing – characterised by home visiting, complex caseloads, palliative care, and high clinical autonomy.
