Key responsibilities

• Manage a defined caseload within a CMHT setting
• Carry out comprehensive mental health assessments
• Support care planning and risk management
• Work collaboratively within multidisciplinary teams
• Maintain accurate clinical documentation

Requirements

• Registered Mental Health Practitioner (RMN or equivalent)
• Experience within CMHT or community mental health services
• Strong assessment and case management skills
• Ability to work independently within a community setting
