Join the team as a Continuing Healthcare Nurse and play a key role in delivering high-quality CHC services. In this role, you will:
• Facilitate multidisciplinary assessments and patient reviews
• Complete DSTs, checklists, FNC reviews, and fast-track assessments
• Manage CHC cases in line with the National Framework
• Support care planning and personal health budget pathways
• Work closely with internal teams and external stakeholders
To be successful in this role, you will need:
• A current and valid NMC registration and pin
• Experience working within Continuing Healthcare (CHC)
• Strong understanding of the CHC Framework and assessment process
• Excellent communication and stakeholder management skills
• Ability to work independently and manage caseloads effectively
This position requires full-time availability, Monday to Friday. The contract is ongoing with a strong likelihood of extension.
