Community Mental Health Practitioner (OT / Social Worker / RMN)
Pay rate: £24 – £26 per hour
Hours: 37.5 hours per week (9am–5pm, Monday–Friday)
Duration: ASAP

Overview:
We are supporting a Community Mental Health Team (CMHT) in Chorley with a requirement for an experienced Band 6 Mental Health Practitioner. This role sits within an established multidisciplinary team and involves managing a defined caseload of service users with a range of mental health needs.
Key responsibilities:
• Carry out keyworking duties for a caseload of patients within the CMHT
• Complete assessments, care planning, and ongoing reviews
• Administer depot medications where required
• Demonstrate strong working knowledge of medications including Clozapine and Lithium
• Work collaboratively within a multidisciplinary team
• Maintain accurate clinical records and documentation
Requirements:
• Qualified Occupational Therapist, Social Worker or RMN
• Relevant mental health experience within a CMHT setting
• Experience administering depot injections
• Knowledge of high-risk medications including Clozapine and Lithium
• HCPC / NMC / SWE registration (as applicable)
