We are seeking an experienced and skilled Band 6 Community Psychiatric Nurse (CPN) to join the Wiltshire North Primary Care Liaison Team, based out of Green Lane Hospital in Devizes.
This role features a consistent Monday-to-Friday schedule (9–5), offering an excellent opportunity for a qualified practitioner looking for full-time hours within a specialized primary care liaison setting.
Provide expert mental health triage, comprehensive assessments, and brief interventions within a Primary Care Liaison framework.
Work collaboratively alongside GPs, primary care professionals, and secondary mental health services.
Manage a varied caseload of service users with diverse mental health needs.
Ensure accurate and timely clinical documentation using the RiO electronic patient record system.
Qualification: Registered Mental Health Nurse (RMN) with current NMC registration.
Systems Knowledge: Previous experience using RiO and an active Smartcard (essential).
Experience: Proven experience operating as a Band 6 CPN or within primary care liaison/community mental health services.
Availability: Ready for an immediate start (ASAP).
