Job Title: Band 6 Care Coordinator (CPN)
Location: South London
Duration: Ongoing Contract
Schedule: Monday – Friday | 9:00am – 5:00pm
Start Date: ASAP
Rate of Pay: £27 per hour

What You’ll Be Doing:

Join a specialist community mental health team and play a key role in delivering high-quality, person-centred care to service users with complex mental health needs. In this role, you will:

• Manage and coordinate care for service users with complex mental health presentations
• Complete comprehensive mental health and risk assessments
• Develop, implement, and review person-centred care plans
• Support service users through evidence-based, recovery-focused interventions
• Work collaboratively within a multidisciplinary team
• Maintain accurate and timely clinical documentation
• Liaise effectively with carers, families, and partner agencies
• Participate in team duty responsibilities where required
• Promote safeguarding and positive clinical outcomes
• Utilise EPJS systems for case management and documentation

Requirements & Qualifications:

To be successful in this role, you will need:

• A current and valid NMC registration
• Previous experience as a Community Psychiatric Nurse or Care Coordinator
• Strong community mental health experience
• Proven ability to manage caseloads effectively
• Excellent assessment, triage, and risk management skills
• Desirable: Experience using EPJS
• Strong safeguarding knowledge
• Excellent communication and MDT collaboration skills
• Ability to work autonomously within a community setting
• Immediate availability preferred

Working Hours & Contract Duration:

This role offers full-time hours, Monday to Friday, with an ongoing contract and strong potential for long-term extension.
