What You’ll Be Doing:

Join our team as a Care Coordinator and play a key role in supporting adults with complex mental health needs within the community. In this role, you will:

• act as Care Coordinator for a defined caseload under the CPA framework
• complete comprehensive mental health assessments and develop care plans
• deliver recovery-focused interventions within community settings
• provide holistic care including medication management, physical health monitoring, and depot administration
• carry out risk assessments and crisis interventions
• work collaboratively with MDTs, GPs, and partner agencies
• maintain accurate and timely clinical documentation

Requirements & Qualifications:

To be successful in this role, you will need:

• a current and valid NMC registration and pin
• experience working within CMHT settings
• experience with depot medication administration
• strong communication and interpersonal skills
• the ability to work independently and within a multidisciplinary team
• a commitment to delivering high-quality patient care

Working Hours & Contract Duration:

This role offers a full-time position. The contract is for 12 weeks, with a strong possibility of extension based on service needs.
