£26 per hour
Full Time | Monday – Friday | 09:00 – 17:00

Hunter Mental Health are supporting Gloucestershire Health and Care NHS Foundation Trust in their search for an experienced Band 6 Community Psychiatric Nurse to join the Older Adults Community Mental Health Team based at Avon House, Tewkesbury.

This is an excellent opportunity for an experienced RMN with Later Life experience to join a well-established community team supporting older adults presenting with both organic and functional mental health difficulties.

Role Overview

As part of the Later Life Community Mental Health Team, you will provide comprehensive mental health assessments, risk assessments, care planning and evidence-based interventions for older adults with complex mental health needs. You will work collaboratively with service users, carers, primary care services and multidisciplinary teams to promote recovery, independence and the least restrictive care possible.

Key Responsibilities

• Undertake comprehensive mental health and risk assessments
• Manage and coordinate care for older adults with organic and functional mental health conditions
• Develop, implement and review person-centred care plans
• Act as Care Coordinator where required under CPA
• Provide medication support, monitoring and management
• Work closely with carers, GPs, community services and wider MDT colleagues
• Participate in duty and triage functions within the team
• Support junior staff and contribute to clinical supervision
• Maintain accurate clinical records and documentation

Requirements

• Registered Mental Health Nurse (RMN) with active NMC registration
• Band 6 Community Mental Health experience
• Previous Later Life / Older Adults Mental Health experience is essential
• Strong assessment, risk management and care coordination skills
• Community Mental Health Team experience
• Full UK driving licence and access to transport
• Ability to work independently and as part of a multidisciplinary team
