Join the Older Adults CMHT and play a vital role in supporting older adults with complex mental health needs in the community. In this role, you will:
• Manage a defined caseload as Care Coordinator under the CPA framework
• Complete comprehensive mental health and risk assessments for older adults
• Develop and review person-centred care plans
• Deliver recovery-focused interventions within community settings
• Support service users with dementia, functional mental health conditions, and complex psychosocial needs
• Provide medication management and monitor mental and physical wellbeing
• Carry out safeguarding assessments, crisis planning, and relapse prevention
• Work closely with MDT colleagues, GPs, families, and external agencies
• Maintain accurate, timely clinical documentation
To be successful in this role, you will need:
• A current and valid NMC registration
• Previous CMHT experience
• Older Adults mental health experience
• Strong assessment, care planning, and risk management skills
• Excellent communication and teamwork abilities
• A commitment to delivering high-quality, compassionate patient care
This role offers 30 hours per week for an initial 12-week contract, with strong potential for extension based on service requirements.
