Pay Rate: £280–£300 per day (Inside IR35)
Hours: Full-time
Duration: Initial 3-month contract
Location: Birmingham & Solihull (Hybrid)
Overview:
We are recruiting an experienced Band 6 Registered Nurse to join the All Age Continuing Healthcare (AACHC) Team within Birmingham & Solihull. This is a hybrid position, with all Continuing Healthcare assessments undertaken face-to-face across Birmingham and Solihull, while assessment write-ups can be completed either from home or within the office.
The successful candidate will undertake comprehensive NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs, alongside reviews, case management and multidisciplinary working in line with the National Framework for NHS Continuing Healthcare.
Key Responsibilities:

  • Complete face-to-face CHC and FNC assessments.
  • Undertake Decision Support Tool (DST) assessments and prepare recommendations.
  • Manage a complex caseload including new referrals, reviews, Fast Track assessments and appeals.
  • Work collaboratively with MDTs, Local Authorities, acute trusts, community teams and care providers.
  • Present cases to Continuing Healthcare Panels where required.
  • Produce high-quality assessment reports within national timescales.

Requirements:

  • Current NMC Registration.
  • Previous experience completing NHS Continuing Healthcare (CHC) assessments.
  • Strong working knowledge of the National Framework for NHS Continuing Healthcare, Decision Support Tool (DST), Fast Track and Funded Nursing Care (FNC).
  • Experience managing patients with complex physical health, mental health, learning disability or palliative care needs.
  • Excellent assessment, report writing and multidisciplinary working skills.
  • Ability to travel across Birmingham and Solihull to undertake face-to-face assessments.
