Pay Rate: £25.70 per hour
Contract: Full-time, Locum
Hours: 37.5 hours per week, Monday to Friday, 9:00am – 5:00pm

Hunter MH are recruiting for an experienced Band 6 CPN / Key Worker to join the Community Mental Health Team (CMHT) in Witney. This is a fantastic opportunity for a community mental health professional looking to join a supportive team and manage a varied caseload.

What You’ll Be Doing

• Managing a caseload of adults with a range of mental health needs.
• Completing comprehensive mental health assessments.
• Developing, implementing and reviewing person-centred care plans.
• Providing ongoing care coordination and therapeutic interventions.
• Undertaking risk assessments and risk management planning.
• Liaising with multidisciplinary teams, partner agencies and carers.
• Maintaining accurate clinical records and documentation.
• Supporting service users to promote recovery and independence.

Requirements

• Registered Mental Health Nurse (RMN) with a valid NMC registration.
• Previous experience working within a Community Mental Health Team (CMHT).
• Strong assessment and care coordination skills.
• Experience managing a community caseload.
• Excellent communication, documentation and clinical decision-making skills.

Working Hours & Contract

• Monday to Friday
• 9:00am – 5:00pm
• 37.5 hours per week
• ASAP start
• Ongoing contract
