Duration: Ongoing Contract
Schedule: 37.5 Hours Per Week | Monday – Friday

What You'll Be Doing:

Join a supportive Community Mental Health Team providing recovery-focused care to adults with severe and enduring mental health conditions. In this role, you will:

• Manage a defined caseload of service users within the community setting
• Undertake comprehensive mental health assessments, care planning, and care coordination
• Coordinate and facilitate Care Programme Approach (CPA) reviews
• Develop, implement, and evaluate person-centred care plans
• Complete risk assessments and risk management plans for service users presenting with complex mental health needs
• Respond to crisis situations and provide timely interventions where required
• Work collaboratively with multidisciplinary teams, inpatient services, primary care, social services, addictions services, and partner agencies
• Support service users in their recovery journey, promoting independence, wellbeing, and social inclusion
• Recognise and respond to safeguarding concerns in accordance with Trust policies
• Monitor physical health needs and ensure appropriate escalation and intervention
• Maintain accurate electronic patient records, reports, and clinical documentation
• Participate in clinical supervision, reflective practice, and service development activities
• Provide leadership, support, and guidance to junior staff where appropriate

Requirements & Qualifications:

To be successful in this role, you will need:

• Current and valid NMC, HCPC, or Social Work England registration
• Significant experience working within Community Mental Health Services
• Previous experience managing a caseload and acting as Care Coordinator
• Strong understanding of the Care Programme Approach (CPA)
• Knowledge of the Mental Health Act, Mental Capacity Act, Community Care legislation, and Community Treatment Orders (CTOs)
• Experience completing mental health assessments, risk assessments, and care planning
• Excellent communication and multidisciplinary working skills
• Experience working with adults with severe and enduring mental health conditions
• Understanding of recovery-focused practice and social inclusion principles
• Ability to work autonomously and prioritise workload effectively
• Community experience desirable
• Immediate availability preferred

Working Hours & Contract Duration:

This role offers 37.5 hours per week, Monday to Friday, with an ongoing contract and strong potential for long-term extension.
