Hunter MH are seeking a dynamic and compassionate Community RGN - Band 5 to play a pivotal role in delivering personalized, holistic care to a diverse population.
In this role, you'll:
- Conduct comprehensive patient assessments, developing and implementing individualized care plans.
- Provide a full spectrum of nursing care, including health education, immunizations, wound care, and medication management.
- Provide nursing care to patients suffering from a wide variety of medical conditions and illnesses, supporting in their treatment, recovery, and well-being.
- Advocate for your patients, promoting access to essential resources and services.
- Contribute to a positive and welcoming clinic environment for patients and colleagues.

Requirements
- NMC registered nurse
- Current resident and right to work in the UK
- Highly experienced Community Band 5 nurse
- Competent using System 1 software.
- Assess patient and carers needs through strength-based assessment.
- Develop individual care plans in partnership with patients and carers. This will include information, advice and education.
- Regularly evaluate care plans to ensure they are up to date and appropriate, reporting clinical changes or areas of concern to the appropriate member of the team.
- Provide skilled nursing care interventions, support and advice to colleagues, patients, and carers.
- Work to reduce unwarranted variation in clinical practice, providing essential care to patients in their homes.
