Job Title: Community Psychiatric Nurse (CPN) - Triage Process Specialist
Band: 6
Location: Adults Community Mental Health Team (CMHT), Warwick
Hours: Full-time, Monday to Friday, 9:00 am to 5:00 pm

Job Description:

We are seeking a skilled and experienced Community Psychiatric Nurse (CPN) to join our Adults Community Mental Health Team (CMHT) based in Warwick. This is a Band 6 role, focused on supporting the Locality Pathway Allocation Coordinator with clinical oversight of the triage process for routine referrals.

Key Responsibilities:

  • Provide clinical oversight and expertise in the triage of routine referrals, ensuring that the appropriate care pathways are identified for patients.
  • Work closely with Band 4 Clinical Support Assistants, who will complete initial triage assessments, to ensure accurate and timely decision-making.
  • Collaborate with the Locality Pathway Allocation Coordinator to maintain high standards of patient care and streamline referral processes.
  • Utilize Care Notes for documentation and patient record management, ensuring all information is accurately recorded and easily accessible.
  • Conduct regular reviews of triage processes and pathways, offering suggestions for improvement and implementing best practices.
  • Engage with patients, families, and other healthcare professionals to ensure that patients are receiving the most appropriate care and support.
  • Participate in multidisciplinary team meetings to discuss and review patient care plans.

Essential Requirements:

  • Registered Mental Health Nurse (RMN) with a valid NMC Pin.
  • Previous experience in a triage or similar role within a CMHT setting.
  • Proficiency in using Care Notes for patient documentation and records.
  • Strong clinical assessment and decision-making skills.
  • Ability to work autonomously while maintaining effective communication with the team.
  • A full UK driving license and access to a vehicle for work purposes.
