Schedule: Monday – Friday | 9:00am – 5:00pm

What You’ll Be Doing:
Join a specialist Older Adults Community Mental Health Team and play a key role in delivering high-quality, person-centred care to older adults with complex mental health presentations. In this role, you will:
• Screen, triage, and complete initial assessments of new referrals
• Coordinate care for older adults with a variety of mental health conditions
• Manage and support a caseload using evidence-based, recovery-focused approaches
• Assess, plan, implement, evaluate, and document interventions effectively
• Provide practical and emotional support to carers and family members
• Participate in the team duty rota
• Work collaboratively within an integrated multidisciplinary team
• Contribute to clear care pathways regarding support, treatment, and recovery
• Maintain professional standards and high-quality clinical documentation
• Support service development and ongoing MDT collaboration
• Engage in continuous professional development
Requirements & Qualifications:
To be successful in this role, you will need:
• A current and valid NMC registration or relevant professional registration
• Previous Older Adults CMHT or community mental health experience
• Strong experience in triage, assessment, and care coordination
• Experience supporting older adults with complex mental health needs
• Knowledge and experience using PARIS IT system
• Strong safeguarding and risk management skills
• Excellent communication and MDT working ability
• Ability to work independently and within integrated services
• Immediate availability preferred
Working Hours & Contract Duration:
This role offers full-time hours, Monday to Friday, with a 3-month ongoing contract and strong potential for extension.
