We’re currently recruiting for a Band 6 CPN to join an NHS Adult Mental Health Team in Croydon, South East London.
Role: Adult CPN – Band 6
Pay Rate: £27.00 per hour 
? Setting: Community Mental Health Team
? Hours: Full Time, Monday–Friday, 9am–5pm
? Start Date: ASAP – 6 months ongoing (likely extension)
? Main Duties:

• Act as Care Coordinator for adults with complex and enduring mental health needs within the Community Mental Health Team, working in line with CPA processes.
  • Undertake comprehensive bio-psycho-social assessments, formulate care plans, and deliver evidence-based interventions in line with NICE guidance.
  • Complete and regularly review risk assessments, including crisis and relapse prevention planning, ensuring robust risk management and safeguarding practice.
  • Manage a defined caseload, prioritising workload effectively while responding to urgent and crisis presentations as required.
  • Administer and monitor medication in line with NMC standards, including supporting concordance and liaising with medical colleagues regarding treatment plans.
  • Work collaboratively within a multidisciplinary team, providing clinical expertise, supervision/support to junior staff where appropriate, and contributing to case discussions.
  • Liaise with external agencies including primary care, social services, inpatient units, and voluntary sector organisations to ensure seamless care pathways.
  • Contribute to service development, audit, and quality improvement initiatives to enhance patient outcomes and service delivery.
