What You’ll Be Doing

Join a busy Assessment & Liaison Team providing high?quality, recovery?focused care to adults with complex mental health, physical health and social care needs. In this role, you will:

• Undertake comprehensive mental health assessments, biopsychosocial formulations and risk assessments • Plan, implement and monitor individualised care plans in collaboration with service users, carers and MDT colleagues • Deliver person?centred interventions that promote recovery, wellbeing and health promotion • Administer medication safely, providing clear information on side effects and contraindications • Coordinate and monitor commissioned care packages, ensuring needs assessments are completed and actioned • Provide ongoing follow?up, review and adjustment of care plans based on clinical presentation • Work confidently within legal and professional frameworks including MHA, MCA, CPA, safeguarding and Trust policies • Build therapeutic relationships that support engagement, collaboration and shared decision?making • Maintain accurate electronic patient records and high?quality written communication • Work effectively with MDT colleagues, social care, support groups and external agencies • Contribute to a safe, welcoming clinical environment and escalate risks or hazards appropriately • Participate in service development, audits, evaluations and improvement projects • Provide supervision, mentorship and support to junior staff and students

Requirements & Qualifications

To be successful in this role, you will need:

• Current and valid NMC registration (RMN) • Strong experience within Community Mental Health, Assessment & Liaison or acute mental health settings • Ability to undertake detailed assessments, risk assessments and care planning • Confidence managing a busy caseload and working autonomously • Skilled in therapeutic communication and handling sensitive issues • Understanding of safeguarding, risk management and evidence?based interventions • Knowledge of MHA, MCA, CPA and relevant legislation • Ability to work in a fast?paced environment and manage competing priorities • Strong MDT collaboration skills and experience of interagency working • Basic IT skills and accurate clinical documentation • Immediate availability preferred

Working Hours & Contract Duration

This role offers 37.5 hours per week, Monday to Friday, with an ongoing contract and strong potential for extension.
