Join a supportive Community Mental Health Team providing holistic, recovery?focused care to service users with complex mental health, physical health and social care needs. In this role, you will:
• Manage a community caseload of up to 35 service users with severe and enduring mental health needs • Undertake comprehensive mental health assessments, risk assessments and care planning • Deliver person?centred interventions and support relapse prevention • Coordinate and monitor care packages using complex clinical decision?making • Provide therapeutic activities, emotional support and stability while maintaining professional boundaries • Work collaboratively with MDT colleagues, GPs, social care, inpatient teams and partner agencies • Complete community and home visits as required • Promote shared decision?making with service users, families and carers • Recognise and respond to safeguarding concerns in line with Trust policy • Maintain accurate electronic patient records and documentation • Participate in clinical supervision, reflective practice and service improvement initiatives • Uphold high standards of quality, safety and governance within the CMHT
To be successful in this role, you will need:
• Current and valid NMC registration (RMN) • Strong experience within Community Mental Health Services • Ability to undertake mental health assessments, risk assessments and care planning • Confidence managing a busy caseload and working autonomously • Excellent communication skills with service users, families and MDT partners • Understanding of safeguarding, risk management and evidence?based practice • Ability to travel to site – no remote working • Immediate availability preferred
This role offers 37.5 hours per week, Monday to Friday, with an ongoing contract and strong potential for extension.
