Job Advert: Band 6 Care Co-ordinator (CHMT) – Humber Area

Are you a passionate and experienced mental health professional looking for your next challenge? We have an exciting opportunity for a Band 6 Care Co-ordinator to join our Community Mental Health Team (CHMT) in the Humber area.

About the Role:

As a Band 6 Care Co-ordinator, you will play a pivotal role in providing high-quality, person-centred care to individuals with complex mental health needs. You will manage a caseload, working collaboratively with service users, families, and other professionals to ensure effective care planning and risk management.

Key Details:

  • Pay Rate: £25-£28 per hour, depending on experience.
  • Hours: Full-Time, Monday to Friday, 9:00 AM – 5:00 PM (On-Site Only).
  • Location: Humber Area – Free Parking Available.
  • Start Date: December 2024 – January 2025.
  • Contract Length: 3-6 months, ongoing.

Key Responsibilities:

  • Developing and implementing comprehensive care plans.
  • Conducting risk assessments and ensuring appropriate interventions are in place.
  • Coordinating with multidisciplinary teams to provide seamless and integrated care.
  • Supporting service users in achieving their recovery goals and maintaining their mental well-being.
  • Ensuring timely documentation and adherence to NHS guidelines.

Requirements:

  • Professional Qualification as a Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT).
  • Experience in Mental Health: A proven track record in community mental health services.
  • Skills: Strong organizational, communication, and interpersonal skills.
  • Registration: Current registration with the NMC, HCPC, or relevant professional body.
