Hunter Gatherer Mental Health are currently recruiting for an experienced Band 6 Community Psychiatric Nurse (CPN) to join a Community Mental Health Recovery Service (CMHRS) based in Leatherhead.

Role Details:
Role Title: Band 6 CPN – Community Mental Health
Pay Rate: £25 – £27 per hour (DOE)
Hours: Monday to Friday, 9:00am – 5:00pm
Duration: 3 months ongoing
Location: Leatherhead

Overview:
This is a community-based role within a CMHRS team, supporting adults with complex and enduring mental health conditions. The successful candidate will be responsible for managing a defined caseload and delivering high-quality, recovery-focused care within the community.

Key Responsibilities:

  • Managing a caseload of service users with complex mental health needs
  • Completing comprehensive mental health and risk assessments
  • Developing and reviewing care plans in line with CPA framework
  • Providing care coordination and ensuring continuity of care
  • Supporting service users to maintain independence and recovery
  • Working collaboratively with MDT, GPs, social services, and third-sector providers
  • Maintaining accurate and timely clinical documentation

Requirements:

  • Registered Mental Health Nurse (RMN) with active NMC registration
  • Previous experience within a CMHT / CMHRS setting
  • Strong risk assessment and care coordination skills
  • Ability to work independently within a community setting
  • Car driver preferred due to travel requirements
