In this role, you will deliver accessible, non?stigmatising, localised community care to adults with established mental health needs. Responsibilities include:
Leading the clinical management of a defined caseload.
Assessing, planning, implementing and evaluating nursing care using complex clinical decision?making.
Delivering holistic mental and physical health assessments, including physical observations (BP, pulse, respirations, blood sugars, urinalysis, weight/height).
Providing individual and group therapeutic interventions, including CBT?informed approaches.
Ensuring person?centred care packages are coordinated, monitored and regularly reviewed.
Responding to changes in service users’ mental or physical health and offering timely crisis intervention.
Completing risk assessments and risk management plans in line with SABP policies.
Recognising and responding to safeguarding concerns for vulnerable adults and children.
Maintaining accurate, high?quality clinical documentation consistent with SABP record?keeping standards.
Working collaboratively with MDTs, acute teams, social care, primary care, voluntary sector partners and external agencies.
Supporting therapeutic engagement, emotional stability and professional boundaries.
Mentoring, supervising and teaching junior staff and students.
Contributing to service improvement, audit, benchmarking and quality monitoring.
Upholding SABP values and promoting a positive image of the team and Trust.
Ensuring compliance with health, safety, security and risk policies.
Supporting the CPA process, including care planning and inter?agency liaison.
Ensuring safe custody and supervision of service users in accordance with leave status.
NMC RMN registration
Mentorship qualification (or willingness to undertake)
Minimum 2 years post?qualification experience
Experience delivering community?based mental health care
Strong skills in assessment, risk management, care planning and MDT collaboration
Understanding of evidence?based practice, safeguarding, CPA and recovery?focused care
Ability to work autonomously and make complex clinical decisions
Awareness of professional boundaries, clinical supervision and reflective practice
Full UK driving licence and access to a vehicle (community visits required)
Immediate availability preferred
Full?time, 37.5 hours per week, Monday–Friday. Ongoing contract with strong potential for extension.
