Duration: 3 Month Ongoing Contract
Schedule: 28 Hours per Week | 4/5 Days per Week | Monday – Friday | 9:00am – 5:00pm

What You'll Be Doing:
Join a Specialist Dementia Frailty Team and play a key role in supporting adults living with dementia and complex mental health needs within the community. In this role, you will:
• Manage a caseload of patients with dementia and associated mental health needs
• Undertake comprehensive mental health and dementia-focused assessments
• Monitor risk, wellbeing and ongoing care needs
• Develop, implement and review person-centred care plans
• Work closely with service users, carers and families to provide holistic support
• Liaise effectively with GPs, social care professionals and wider healthcare services
• Participate as part of a multidisciplinary team to deliver high-quality care
• Maintain accurate and timely clinical records using electronic systems
• Travel across the geographical area to conduct community-based visits
Requirements & Qualifications:
To be successful in this role, you will need:
• Current NMC registration as a Mental Health Nurse
• Previous experience working with people with dementia and mental health needs (essential)
• Community Mental Health or CPN experience
• Full UK driving licence and access to a vehicle (essential)
• Strong communication and interpersonal skills
• Good IT and electronic record-keeping skills
• Ability to work independently and use initiative appropriately
• Experience working within multidisciplinary teams
Working Hours & Contract Duration:
This role offers 28 hours per week across 4 or 5 days, Monday to Friday, with working days agreed at interview. The position is available immediately and is offered on an initial 3-month ongoing contract with strong potential for extension.
