Hunter Mental Health are currently recruiting for an experienced Band 6 Community Mental Health Nurse (CPN) to join a Working Age Adults Community Mental Health Recovery Service in Leatherhead.

This is an excellent opportunity to join a busy Community Mental Health Team, providing assessment, care coordination and recovery-focused interventions for adults with complex mental health needs within the community.

Duties

  • Manage a caseload of working age adults with complex mental health needs.
  • Undertake comprehensive mental health and risk assessments.
  • Develop, implement and review person-centred care plans.
  • Coordinate CPA reviews and work collaboratively within the multidisciplinary team.
  • Liaise with GPs, carers and external agencies to promote positive recovery outcomes.
  • Maintain accurate clinical documentation and electronic patient records.

Requirements

  • Registered Mental Health Nurse (RMN) with a valid NMC registration.
  • Previous Community Mental Health Team (CMHT) / CPN experience is essential.
  • Full UK driving licence and access to a vehicle.
  • Strong assessment, risk management and care coordination skills.
  • Available to start as soon as possible.
