Overview

Hunter Mental Health are supporting a Community Mental Health Recovery Team in Stroud and are looking for an experienced Band 6 Community Mental Health Nurse to join the service.

The successful candidate will work with adults experiencing severe and enduring mental health conditions, supporting recovery, social inclusion and community-based care. This is a community role involving patient visits and multidisciplinary working across local mental health services.

Key Responsibilities

  • Managing a caseload of community mental health patients
  • Completing comprehensive mental health and risk assessments
  • Acting as Care Coordinator under the Care Programme Approach (CPA)
  • Developing and reviewing person-centred care plans
  • Supporting service users with recovery-focused interventions
  • Liaising with families, carers and partner agencies
  • Participating in MDT meetings and duty functions
  • Maintaining accurate clinical records and documentation
  • Contributing to safeguarding, risk management and crisis planning activities

Requirements

  • Registered Mental Health Nurse (RMN) with valid NMC registration
  • Previous Community Mental Health experience
  • Experience undertaking risk assessments and care coordination
  • Knowledge of CPA processes and recovery-focused practice
  • Strong communication and multidisciplinary working skills
  • Full UK driving licence and access to transport preferred due to community visits
