Locations: Manchester | Salford | Trafford | Bolton | Wigan (Candidates must be flexible to work across sites within their chosen locality. If you are happy to relocate, please make this clear.)
 
Duration: 6?Month Ongoing Contract (Strong potential for extension)
 
Schedule: 37.5 Hours Per Week | Monday – Friday

What You’ll Be Doing
Join a supportive Adult Community Mental Health Team delivering safe, personalised, trauma?informed and recovery?focused care to adults with serious mental health needs. In this role, you will:
• Manage a defined caseload within the Adult CMHT pathway
• Complete biopsychosocial assessments, triage and formulation
• Develop co?produced, personalised care and support plans aligned to the Personalised Care Framework (PCF)
• Deliver brief interventions, psychosocial work, relapse prevention and recovery?focused support
• Provide Named Worker responsibilities, ensuring continuity, safety planning and outcome?focused reviews
• Use DIALOG+ and other outcome measures to support meaningful conversations and shared decision?making
• Respond to crisis presentations, deterioration and emerging risks, escalating appropriately
• Complete collaborative risk assessments, safety plans and safeguarding referrals
• Work flexibly across referral, assessment, intervention, review, recovery and discharge functions
• Liaise with GPs, PCNs, local authority teams, VCFSE partners, inpatient/crisis services and wider GMMH pathways
• Support assertive outreach for individuals with complexity, disengagement or heightened risk
• Maintain accurate, timely and high?quality clinical documentation
• Participate in MDT huddles, weekly MDT meetings, reflective practice and supervision
• Contribute to service improvement, audit, incident learning and quality assurance
 
Requirements & Qualifications
Band 6
• RMN / OT / Mental Health Social Worker
• Experience within community mental health or equivalent complex settings
• Skilled in assessment, formulation, care planning, risk management and MDT collaboration
• Strong communication skills in emotionally heightened situations
• Ability to work autonomously and manage competing demands
• Willingness to work flexibly across multiple localities
• Immediate availability preferred
 
Working Hours & Contract Duration
This role offers 37.5 hours per week, Monday to Friday, with a 6?month ongoing contract and strong potential for long?term extension.
