Band 6 Community Mental Health Nurse – Dementia Services
Location: Oswestry, Market Drayton & Severnfields (Shrewsbury)
Duration: 12 Weeks Initially
Schedule: Monday – Friday, 09:00 – 17:00
Start Date: ASAP
Rate of Pay: £26 per hour

Hunter Gatherer Mental Health are currently recruiting for a Band 6 Community Mental Health Nurse to join a Dementia Service based across Oswestry, Market Drayton and Severnfields, Shrewsbury. This is a fantastic opportunity for an experienced clinician with a strong background in supporting older adults living with dementia within a community setting.

What You’ll Be Doing

  • Managing a complex caseload of service users living with dementia within the community.
  • Supporting the referrals management process and triaging new referrals.
  • Providing intensive support and interventions for service users and carers with complex needs.
  • Delivering holistic, person-centred care aimed at improving quality of life.
  • Undertaking assessments, care planning, risk management and reviews.
  • Working autonomously whilst contributing effectively within a multidisciplinary team.
  • Delivering a range of therapeutic interventions and evidence-based practice.
  • Supporting and supervising junior staff where appropriate.
  • Contributing positively to service development and continuous improvement initiatives.

Requirements & Qualifications

To be successful in this role, you will need:

  • Registered Mental Health Nurse (RMN) qualification with current NMC registration.
  • Previous experience working with older adults living with dementia.
  • Community mental health experience at Band 6 level or equivalent.
  • Experience managing complex caseloads independently.
  • Strong assessment, risk management and care planning skills.
  • Previous supervisory experience.
  • Excellent communication and organisational abilities.
  • Ability to work effectively under pressure within a fast-paced service.
  • Full UK driving licence and access to transport is desirable.

Working Hours & Contract Duration

This is a full-time position working Monday to Friday, 09:00–17:00, with an initial 12-week contract and the possibility of extension depending on service requirements.
