We have an excellent role available within Southwark CMHT, offering stable hours, strong MDT support, and the chance to make a meaningful impact on service users’ recovery and independence.

Job Title: Band 6 Care Coordinator – Community Mental Health Team
Location: Southwark
Duration: Ongoing Contract
Schedule: Monday–Friday, 9am–5pm (37.5hrs)
Start Date: ASAP
Rate of Pay: £27ph

What You’ll Be Doing:

Join a neighbourhood?based CMHT providing recovery?focused, person?centred support. In this role, you will:

  • Manage a defined caseload as a registered Mental Health Practitioner
  • Complete holistic mental health assessments and develop recovery?focused care plans
  • Coordinate care across MDTs, inpatient teams, interface services and external agencies
  • Lead regular reviews, ensuring care plans remain responsive and goal?driven
  • Support service users to build independence, self?care and social inclusion
  • Recognise and escalate physical health concerns for service users with long?term conditions
  • Carry out risk assessments, crisis planning and risk management interventions
  • Respond appropriately to urgent situations and follow lone?working protocols
  • Implement safeguarding procedures and Duty of Candour requirements
  • Maintain accurate electronic clinical records, reports and documentation
  • Administer and monitor medication (RMNs only) in line with NMC standards
  • Provide leadership within the team and support junior colleagues
  • Contribute to service improvement, audits and performance standards

Requirements & Qualifications:

To be successful in this role, you will need:

  • Current registration with NMC, HCPC or Social Work England
  • Professional qualification in Mental Health Nursing, Social Work, OT, Psychology or allied profession
  • Relevant post?registration mental health experience
  • Experience working within MDTs and supporting complex service users
  • Strong skills in assessment, care planning, risk management and crisis response
  • Knowledge of mental health legislation, CPA processes and clinical governance
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and manage a caseload effectively
  • Experience in community mental health (desirable)
  • Immediate availability preferred

Working Hours & Contract Duration:

This is an ongoing locum opportunity offering stable Monday–Friday hours within a supportive CMHT environment.
