Join a borough?wide SPA service providing first?line triage, assessment and wellbeing support. In this role, you will:
• Carry out mental health triage and structured risk assessments using SPA proformas • Lead conversations about wellbeing, self?management and appropriate support pathways • Provide clear clinical direction on whether needs are best met in primary care, secondary care, or specialist triage services • Signpost to Living Well Network offers, community groups, charities and universal services • Liaise daily with MDT colleagues, GPs, social care, police, emergency services and partner organisations • Ensure continuity of care through excellent communication and collaborative working • Maintain high?quality electronic documentation and uphold data protection standards • Recognise safeguarding concerns and escalate appropriately • Support service users and carers to understand available services and navigate the system • Contribute to service development, cross?organisational working and quality improvement • Act as a positive clinical role model and support junior staff where required • Deputise for the Team Leader or Clinical Expert when needed
To be successful in this role, you will need:
• Current NMC registration as RMN • Minimum 12 months post?qualification experience • Experience in community mental health or acute inpatient settings • Strong skills in mental health assessment, triage and risk formulation • Ability to make sound clinical decisions under pressure • Excellent communication skills and confidence leading wellbeing conversations • Knowledge of mental health legislation, safeguarding and CPA processes • Experience working within multidisciplinary and multi?agency environments • Commitment to person?centred, strengths?based practice • Immediate availability preferred
