Location: Reading Area
Salary: £25PH to £27PH, depending on experience
Contract: Full-time or Part-time, Immediate Start Available
Remote Working: Partially Remote
Car Required: Yes, for community travel

Are you a dedicated Band 6 Community Psychiatric Nurse (CPN) or Social Worker looking to make a meaningful difference? A great client of ours is seeking a professional for an Adult Community Mental Health Team, supporting individuals aged 18 to 65 in the Reading area. They offer a flexible work environment with a balance of on-site and remote working.

Role Overview

You will be responsible for managing a caseload of up to 30 service users. You’ll play a vital role in care coordination, delivering high-quality support and interventions tailored to each individual’s needs.

Key Responsibilities

  • Manage a caseload of up to 30 people aged 18–65 with diverse mental health needs
  • Provide care coordination and case management within the community
  • Deliver person-centered assessments, interventions, and care plans
  • Work collaboratively with other healthcare and social care professionals
  • Attend team meetings and case discussions
  • Engage with clients and their families to promote recovery and wellbeing

Requirements

  • Valid NMC registration for CPN or SWE registration for Social Workers, with no restrictions
  • Experience in community mental health, ideally in an adult setting
  • Ability to manage a caseload efficiently, maintaining a high standard of care
  • A car and valid driver’s license to travel within the community
  • Strong interpersonal and communication skills
