We're currently recruiting on behalf of a well-established NHS Mental Health Trust in Manchester for an experienced CPN/Care Coordinator to join their community team. This is an excellent opportunity to manage a complex caseload, coordinate person-centred care and work collaboratively within a multidisciplinary service.
Role: Band 6 CPN – Community Care Coordinator
Pay Rate: £26 per hour
? Location: Manchester
? Hours: Monday–Friday, 9am–5pm
? Start Date: ASAP – 2 months
? Main Duties:
Manage an allocated caseload of service users with complex mental and physical health needs within the community.
Complete comprehensive clinical assessments, care planning and risk assessments.
Coordinate and evaluate person-centred interventions in line with clinical pathways.
Participate in the CMHT duty rota, responding appropriately to changes in service users' mental health.
Provide professional advice and support to junior and support staff.
Undertake depot clinics where required.
Work collaboratively within a multidisciplinary team (MDT) to deliver effective care.
Contribute to the planning, development and evaluation of the CMHT service.
Provide supervision and support to students on clinical placement.
Maintain accurate clinical documentation in line with professional and Trust standards.
Promote recovery, independence and person-centred care.

Essential Requirements:
Current professional registration as a Registered Mental Health Nurse (RMN).
Strong experience working within community mental health services.
Experience managing a caseload and coordinating care.
Strong clinical assessment and risk management experience.
Ability to work effectively within a multidisciplinary team.
Strong communication, organisation and clinical decision-making skills.
Ability to manage complex mental health presentations in a community setting.
